Diabetes pilot program interest survey

Please fill out this form if you are interested in joining the pilot program for DHF's Diabetic Prevention & Management program. The pilot program will consist of a select group of Prime associates who fall into diabetic or pre-diabetic blood glucose ranges and are ready to make changes to reach better health. The Diabetic Prevention & Management program will be a 12 week program focusing on education and lifestyle change to improve health and blood glucose. The program will be in a follow-along format with all of the tools and resources available 24/7 on your phone through our website driverhealthandfitness.com.
