General client/driver info
This coaching form is to be used for the third coaching call for the Fit in 15 weight control program. The DHF staff member will fill this out during the health coaching call and then submit so we can keep track of each driver's goals throughout the program.
Decisional Balance Exercise
Discuss with the driver what he/she perceives to be the short-term and long-term benefits and drawbacks of making healthy changes. Encourage the driver to generate personal costs and benefits, and then openly discuss these to identify barriers and goals.
This concludes the third health coaching call. Try to sign the driver up for their last coaching call over the phone spaced 2 weeks out or more.
